Request Paid Time Off

Employees are entitled to use paid time off in accordance with the New Jersey Sick Leave law. In order to recieve paid time, an employee must submit a Paid Time Off request. Completed forms are to be submitted no later than payroll Monday by 5:00 pm. for the pay period in which you are looking to be paid. Requests submitted later than that date may not be included with the payroll for that pay period. Employees may not request PTO or be paid for any day in which they work and can only be paid for accrued time. The maximum accrual for any year is 40 hours and the maximum that an employee may be paid in any given year is 40 hours.

To take PTO for personal use, not related to legitimate, unexpected illness or emergencies, it requires one month advance notice to the scheduling department and must be approved in advance. Avenues appreciates as much notice as possible when you know you expect to miss work for a scheduled absence.


If taking off in multiple pay periods, please use only one form per pay period.
